It’s actually a still image from an Italian movie “Oggi a Berlino” (aka “East Zone, West Zone”), directed by Piero Vivarelli in 1962.

The viral caption is misleading, because it fails to mention that the source of this image is a fictional movie.

Good question. I don’t know if anything similar happened. Of course there were plenty of incidents. Examples:
